34 year old hispanic female with 6 month history of Type 2 diabetes presents in follow up.  She lowered her A1c from 11% to 8% on combination of metformin 1000 mg BID and glipizide 5 mg BID.  She had diabetes education upon diagnosis. Since diagnosis, she has gained 12 pounds.  She reports great difficulty with needle phobia.  Because of this, she tests her BG very infrequently.

ROS:  Recurrent vaginal yeast infections

PE:  Healthy obese woman.  Wt – 227 lbs, Ht – 65”, BMI – 37.8 , BP – 128/72, P – 80.  Acanthosis nigricans on back of neck.  Otherwise normal.

Lab:  8.0%, TC – 215 mg/dl, TG 227 mg/dl, HDL-C – 42 mg/dl, LDL - C – 127 mg/dl, eGFR - >60, urinary albumin level – 8 mg/grCr.
